Use this form if you wish to use our facilities. Restrictions apply. Contact the office for more information.
Work Order form
If you need an area of the building setup for a meeting or event fill out this form. If you fill out a facility request form, or if you have a class or event that occurs every week you need to fill this form out as well.
My Story
If you wish to be baptised and have already visited the Hospitality Room, you can fill out this form.
Background Check Consent form
If you wish to work with the children or students, you must fill out this form so that we can complete a criminal background check on you.
